The survey comes in the category of the quizzes so you can change the name of any Quiz to Survey and start receiving the input from the users.
Follow some basic steps to change quiz to survey:
Go to Learndash Settings
Now make some changes to the Post Names by going to Custom Labels Option and Change Quiz -> Survey and Quizzes -> Surveys.
After making changes you will see the post name like this:
Finally, you can go in there and add questions there with the options.
You will see everywhere Written Survey instead of Quiz.
